2. Purpose of Consent

We would love to share your story to inspire others and demonstrate the impact of our work. Your story may be used in our fundraising and marketing communications, including: ✔ Website and social media ✔ Printed materials (e.g, brochures, newsletters, annual reports) ✔ Email campaigns and press releases ✔ Fundraising appeals

We will always handle your information with care and respect.

You have the right to withdraw consent at any time by contacting MCR Pathways at the contact details at the bottom of this form.

3. Consent for Recording & Story Use

  • I give consent for my story to be used for the purposes described above.
  • I understand that during the collection of my story through interviews these may be recorded for the purposes of ensuring the written copy created is accurate, but that after I have agreed to the story the recording will be deleted.
  • I understand my story may be edited for clarity or to be used for different purposes while keeping my words and experiences authentic.
  • I understand my information will be stored securely and will not be shared with third parties without my consent.
  • I will not receive payment for the use of my content.
  • Copyright of all materials will be retained by MCR Pathways
  • MCR Pathways may need to modify content from the original story while maintaining its authenticity and impact and may do so without requiring further approval, based on standard ethical storytelling practices and this consent agreement. This may include modifying for clarity and readability, adjusting for length and format, correcting grammar and spelling, protecting identity while retaining authenticity and adapting the story for different communication channels. If significant changes were needed such as altering the core meaning, adding new details, or using the story in a new, unexpected context MCR Pathways would seek further approval from me.
  • I understand I may be contacted in the future for updates to the story.
  • I understand my story and image will be anonymised for use in MCR Pathways fundraising work, unless I choose below to waive my anonymity

4. Privacy and Data Protection Compliance

  • Your personal data will be processed in accordance with the UK GDPR and Data Protection Act 2018.
  • We will store your consent securely for 5 years, after which it will be securely deleted unless renewed.
  • You have the right to request access, correction, or deletion of your data at any time.
  • You have the right to lodge a complaint with the Information Commissioner’s Office
  • For anyone under 18, names and images will automatically be anonymised.
  • Content published online may be viewed worldwide, including in countries with different privacy standards than the UK/EU
