APCS data breach reimbursement form

Please use this form to receive reimbursement for key documents that you have had to replace as a result of the APCS data breach.

We are collecting and processing your bank account details only for the purpose of processing the reimbursement payments requested below.  We rely on the lawful basis of legitimate interests to collect and process your bank account details for this purpose. It is in our legitimate interests to reimburse you for the costs you have incurred, and we require your bank account details to make the payment.

Your bank account details will be retained for a short period only, and will be securely deleted within seven (7) days of the payment being made.
